JOB SUMMARY

The Statewide Peer Wellness Coach (PWC) will manage the Peer Wellness Initiative for adults across Middle TN who live with mental illness, substance use disorders, or co-occurring disorders. The trainer will oversee the statewide implementation of My Health, My Choice, My Life health and wellness initiative focusing on self-management programming for participants. The PWC will identify barriers to implementation and develop recommendations to address those barriers. The trainer will also serve as a subject matter expert for the state on wellness and chronic diseases and provide technical assistance and consultation to providers as needed.

ESSENTIAL FUNCTIONS

  1. Collaborate with staff, members, and the community to gain the perspectives of others having diverse opinions, abilities, values, beliefs, perspectives, and ethnic or cultural backgrounds.
  2. Take deliberate steps to increase own cultural competency by attending trainings, events, discussions, workshops, etc.

2. The PWC performs the following duties related to becoming a PWC and conducting training and/or workshops:

  1. Provide My Health, My Choice, My Life wellness curricula to participants with fidelity to the evidence-based model.
  2. Collaborate with agency staff of each location at which the My Health, My Choice, My Life initiative is offered to coordinate health and wellness activities, including but not limited to, physical activities, healthy snacks, symptoms and/or stress management activities, health and wellness education, workshop graduation celebrations, brief interventions for Peer Wellness Coaching, other wellness activities, etc.
  3. Participate in trainings that include but are not limited to the Chronic Disease Self-Management Program (CDSMP), Diabetes Self-Management Program (DSMP), Chronic Pain Self-Management Program (CPSMP), Tobacco Free, Matter of Balance (MOB), and Peer Wellness Coaching;
  4. Supplement program curriculum with appropriate materials and guest presenters from local resources;
  5. Participate in Motivational Interviewing (MI) trainings and begins process of becoming certified to provide MI training via Motivational Interviewing Network of Trainers (MINT);
  6. Participate in and/or facilitates trainings as designated by the State in collaboration with the Grantee.

3. The PWC performs the following duties related to participants of the My Health, My Choice, My Life initiative:

  1. Provide Peer Wellness Coaching to participants, utilizing the eight (8) dimensions of wellness to create goals which foster independence and improve health and wellness behaviors;
  2. Develop liaison relationships with case managers and local health care providers and works with participants to establish an ongoing relationship with a primary health care practitioner;
  3. Provide support to clients engaging with primary care physicians as needed.
  4. Identify participants who appear to need individualized assistance, whether due to cultural issues, reading difficulties, or disability, and provides the appropriate assistance; and
  5. Utilize techniques to assist participants (probing, individualized administration, providing context for questionnaire items, Motivational Interviewing) and makes accommodations for them.

4. The PWC performs the following duties related to the MHMCML initiative evaluation:

  1. Participate in a series of training sessions and/or meetings conducted or arranged by the State's Office of Wellness and Employment;
  2. Administer the self-survey to each workshop participant. Participants complete the self-survey individually, and the PWC reports data from the self-survey in the Monthly Reports as directed by the Office of Wellness and Employment.

  4. Inform participants that participation in evaluation activities is strictly voluntary and that declining to participate will not result in discontinuation of services.

5. The PWC performs the following duties related to reporting and other activities:

  1. Meets and complies with all State reporting requirements;
  2. Submit monthly and annual reports to the State’s Office of Wellness and Employment in a format prescribed by the State.
  3. Prior to monthly invoice submission, the PWC shall submit programmatic report(s) to the Office of Wellness and Employment.
  4. Log all health and wellness activities including but not limited to, physical activities, healthy snacks, symptoms and/or stress management activities, health and wellness education, workshop graduation celebrations, brief interventions for Peer Wellness Coaching, other wellness activities, etc., in a format prescribed by the State; and
  5. Assist the State as necessary with all initiative events and activities; and participates in other activities as approved, directed, and authorized by the State
SKILLS AND QUALIFICATONS
  • Be a current or past recipient of mental health services, and/or co-occurring services;
  • Be a Certified Peer Recovery Specialist, or if not currently a Certified Peer Recovery Specialist become a Certified Peer Recovery Specialist within one (1) year from the date of hire; and
  • Hold at minimum a Bachelor’s Degree from an accredited institution of higher education.

WORKING CONDITIONS

This job operates in a professional office environment. Hybrid remote and in-office opportunities.

This position will work and travel within Davidson County but also provide workshops in surrounding counties (Robertson, Rutherford, Sumner, Williamson and Wilson)
